With effect from 1 November 2022 Citigroup Inc. has transferred ownership of its consumer banking business in Thailand to United Overseas Bank (Thai) PCL (registration number 0107535000176) and/or its related group entities ("UOB").UOB is the issuer of "Citi" branded consumer banking products in Thailand and Citibank, N.A., Bangkok Branch is providing certain services in respect of those products.The trademarks "Citi", "Citibank", "Citigroup", the Arc design and all similar trademarks and derivations thereof are used temporarily under licence by UOB entities from Citigroup Inc.

I agree for United Overseas Bank (Thai) PCL. ("Bank") to contact me using the data I provided in the above form to offer me its products or services. I have read and understood the processing of my personal data which is described in the Bank’s  privacy notice.

I have thoroughly read and comprehended all the information pertaining to the products and/or services, including other relevant details. With this understanding, I would like to proceed by submitting my personal data to apply for the desired product and/or service.

I hereby confirm that I voluntarily provide the Personal Data of my friends or related persons (collectively called "Friend"), which includes full name, telephone number and email, as specified in this page, to Citi to contact my Friend and offer Citi's products and services to my Friend. In this regard, I hereby represent and warranty that:

1. I have informed my Friend that I will provide his/her Personal Data to Citi for Citi to contact and offer Citi's products and services to him/her, and I will provide my Friend with the Privacy Policy of Citi immediately after I have provided his/her Personal Data to Citi;
